This is my final blog post before graduating from chiropractic school this week! I have loved chiropractic school, and I have grown so much as a person in the four years that I have been a student at 911爆料. The professors, students and patients have been amazing, and I am so excited and honored to utilize the tools I have learned to become the best chiropractic physician possible. I learned so much these last ten weeks at my preceptorship in Elmhurst as well. I had many new patient interactions and asked many questions about diagnoses and patient flow in an office. I am especially grateful to my husband, my closest friends, and my family for supporting me through this stressful journey. There were many challenges during the chiropractic program, including COVID-19, but I am so grateful for my time at 911爆料. With all of that being said, I am so excited and motivated to treat patients as a chiropractor in Illinois. I cannot wait to see how much I will have grown in the profession years from now.

I want to thank every person who took the time to read my blogs, and I wish everyone the best of luck during this stressful finals season. I have greatly enjoyed being the chiropractic blogger during my time at 911爆料 and I hope everyone takes a moment (or two) to reflect on their journey in school, as it flies by quickly. At 911爆料, we learn many different chiropractic techniques that can benefit a variety of patient presentations. Recently, I shadowed a 911爆料 Upper Cervical Chiropractic Association (NUCCA) chiropractor in order to widen my knowledge regarding the different methods utilized within chiropractic medicine.

When the cranio-cervical junction is injured, the NUCCA technique works to reduce nerve interference using a precise, gentle and non-invasive spinal adjustment. By using objective X-rays of the head and neck, along with mathematical analysis, a misalignment pattern can be determined for each unique patient.

After determining the misalignment pattern, the doctor will apply a light pressure that will move the spinal bones in the cervical region. There is no discernible pushing or twisting during the adjustment. This subtle process may be comforting to some patients that feel anxious when they hear 鈥減ops and cracks.鈥

In 2007, research was conducted, concluding that restoration of Atlas alignment was associated with marked and sustained reductions in blood pressure, comparable to the use of two-drug combination therapy in individuals with elevated blood pressure. I believe one of the most exciting parts of being involved in the world of chiropractic medicine is that there are so many different avenues to benefit patients.

Whether you are a traditional chiropractor, a functional medicine chiropractor, or a NUCCA chiropractor, the goal is to help patients feel healthier and live better lives long-term. I recommend shadowing as many different types of chiropractors as possible while at 911爆料, as you never know what avenue will lead to your future chiropractic career!

Last week at my preceptorship I saw a patient diagnosed with adhesive capsulitis. The patient was confused at the fact that he had severely limited mobility without recent trauma. Adhesive capsulitis, which is more commonly known as frozen shoulder, involves stiffness and pain in the shoulder joint.

Frozen shoulder occurs when the joint capsule thickens and tightens around the shoulder joint, restricting its movement. The risk factors of frozen shoulder include individuals (especially women) 40 and older, individuals who have reduced mobility due to a stroke, rotator cuff injury, or broken arm, and individuals with a systemic disease such as diabetes, hyperthyroidism, hypothyroidism, cardiovascular disease and Parkinson鈥檚 disease.

Frozen shoulder typically develops slowly in three stages. The freezing stage is the first stage in which any movement of the shoulder causes pain, and the shoulder鈥檚 ability to move becomes limited. This stage can last 2-9 months. The frozen stage is the second stage in which pain may decrease, but the shoulder increasingly becomes more stiff. This stage lasts from 4-12 months. The last and final stage is the thawing stage in which the shoulder鈥檚 ability to move begins to improve. This stage lasts from 5-24 months. Usually within 1-3 years symptoms improve.

Treatment involves range-of-motion exercises to obtain greater mobility, and may involve corticosteroids and numbing medications injected directly into the joint. In rare and severe cases arthroscopic surgery is necessary to loosen the joint capsule. As future chiropractic physicians, we can be instrumental in the care of adhesive capsulitis by introducing range-of-motion exercises as well as by mobilization techniques.

Fall is one of my favorite times of the year! Every year, Zach and I go to an apple orchard with some close friends to pick apples and enjoy the beautiful fall weather. We also go to a pumpkin patch and pick pumpkins to finish our fall decorating for the season. This year, the weather was perfect, we all wore our favorite flannel shirts and enjoyed apple cider donuts, fudge, and other goodies from the orchard. After taking mountains of pictures, and finding our perfect apples and pumpkins, one of my favorite fall adventures was concluded.

We always end up with a large number of delicious apples, and making applesauce and apple crisp are two of the easiest ways to utilize them — both are such sweet treats for the fall season! I have included my apple crisp recipe below. I hope you enjoy this perfect fall treat as much as I do:

Apple Crisp

For the Crisp Topping:

  • 戮 cup of old-fashioned oats
  • 录 cup of flour
  • 1 teaspoon of cinnamon
  • 录 cup of white sugar
  • 录 cup of brown sugar
  • 陆 teaspoon of nutmeg
  • 4 tablespoons of cold butter

 

For the Apple Filling:

  • 6 Granny Smith apples, peeled, cored, and thinly sliced 2 teaspoons of fresh lemon juice.
  • 2 teaspoons of fresh lemon juice
  • 录 cup flour
  • 戮 cup of white sugar
  • 陆 cup of brown sugar
  • 1 陆 teaspoons of cinnamon

 

Step One: Preheat the oven to 350掳 degrees and lightly spray an 8 x 8 baking dish.

Step Two: Make the crisp topping and stir until crumbly. Set aside.

Step Three: Place the sliced apples into a large bowl and drizzle with lemon juice. Toss until well-coated.

Step Four: In a small bowl, add the flour, sugar, brown sugar, and cinnamon. Stir until combined. Sprinkle the flour mixture over the apples and toss until the apples are coated. Step Five: Layer the apples into the baking dish. Place the crumbly topping on top!

Step Six: Bake for 35-40 minutes or until golden brown. Enjoy with ice cream on top!

Claudication is most often a symptom of peripheral artery disease. The peripheral arteries are the vessels that deliver blood to the extremities. Intermittent claudication is pain affecting the calf, and less commonly the thigh and buttocks. This pain is primarily induced by exercise and relieved by rest.

When an individual is active, the muscles are typically not receiving enough oxygen due to the damage to the arteries that restrict proper blood flow. Damage to these arteries is usually caused by atherosclerosis, which reduces blood flow due to plaquing. Intermittent claudication is the result of muscle ischemia during exercise caused by obstruction to arterial flow. It is a relatively common condition, with one fifth of the population over age 65 being affected. Other conditions such as nerve root compression, spinal stenosis, hip arthritis, symptomatic Baker鈥檚 cyst, venous claudication and chronic compartment syndrome may mimic the symptoms of intermittent claudication. Absent or reduced peripheral pulses support the diagnosis of intermittent claudication, but many patients have normal peripheral pulses.

Treatment should be utilized to reduce the risk from cardiovascular events and should attempt to reduce the symptoms of claudication. Some methods of reducing the risk of a cardiovascular event include smoking cessation, antiplatelet drugs, statins and control of blood pressure and diabetes. Regular exercise is an important addition to the treatment plan of an individual with intermittent claudication. Chiropractic physicians have an important role with individuals diagnosed with intermittent claudication by helping patients modify their diet and exercise to promote optimum health and wellbeing.

I became a student intern at the 911爆料 clinic in January of this year. I cannot believe that last week was the conclusion of my time as an intern, and that this week is the beginning of my preceptorship in Elmhurst!

I remember being terrified of becoming an intern at the clinic, as I was nervous about putting together all of the information I have learned in chiropractic school. It was also scary to have an actual patient who I knew would depend on me for care and concern regarding their chief complaint.

Fast forward ten months, and my time at the clinic has come to an end. I learned so much from my clinician, Dr. Frank (Dr. Frank Frydrych, Chief DC Clinician), and I also learned a lot about myself in the process. I learned that it is always important to ask questions to fully understand concepts and the reasoning behind certain thought processes and techniques.

I also learned that every single patient is so different, and although we are taught textbook answers for certain patient presentations, patients rarely present in this cookie cutter fashion. I had so many simple and complex patients that I was able to learn from, and I am so grateful that they chose to come to the clinic for care. Although I am just as nervous to start my preceptorship as I was to start at the clinic, I know that the experience will be just as rewarding. I look forward to asking questions and learning even more about myself as a future chiropractic physician before I graduate in December.

While studying psychology in my undergraduate career, I volunteered with a hospice organization for three years. During this time, I realized the importance of supporting individuals at the end of their lives. Although I was a stranger to the individuals I saw on a weekly basis, the presence of another human being was valuable in ensuring that they knew that they were not alone. Many of my friends over the years could never understand how I was able to volunteer with hospice, as they deemed it to be a depressing experience. I believe that my strong religious beliefs, and my thoughts on the circle of life gave me the ability to compartmentalize the saddening portions of the experience, and to remember that we will all reach that part of life where we need extra support and love.

In the research study, 鈥 chiropractors were asked what chiropractic care can offer individuals with a cancer diagnosis. Many of the practicing physicians explained that chiropractic care could help a patient with cancer in terms of their pain relief, empathy, mobility, energy levels, quality of life, sleeping patterns and function. Cancer patients who utilized chiropractic care perceived their benefits to be pain relief, sleep pattern improvement, immune system improvement, an increase in well-being, higher energy levels and psychological reinforcement. Clinical practice guidelines developed by the chiropractic profession include referral to mental health professionals in the presence of mental health concerns. Many individuals with a terminal illness will have mental health complications. Screening for these comorbidities by a chiropractic physician does not need to be complicated or require extensive knowledge. Having a good working relationship with mental health providers is crucial for co-management of individuals with terminal illnesses.

There are many end-of-life issues with regard to the patient, the family and the physician. Open communication and empathy are crucial characteristics for the involved physician to embody. The individual鈥檚 needs, whether it is physical or psychological, need to be addressed through a multidisciplinary team approach. A chiropractic physician has a large part in the palliative care portion of end-of-life care, as pain management and psychological wellbeing can be improved by chiropractic techniques. When comanaging an individual with a terminal illness in my future practice, I will utilize the SPIKES model to ensure the best communicative techniques.

The SPIKES model consists of the setting of the discussion, the perception of the patient鈥檚 set of knowledge, the invitation by the patient to the physician to provide information, the imparting of knowledge, the exploration of emotions, and the strategy and summarization of plans for the future. I will also pull from my past experiences with hospice, as it is important to attempt to separate my emotions from the patient鈥檚 emotions and experience. Separating my emotions and realizing that I am there to benefit the patient鈥檚 symptoms, will be my most important strategy for not only terminal patients, but all patients. The literature shows chiropractic physicians as being greatly beneficial in palliative care, and that is how I see myself as a future practicing physician.

Last, the end-of-life issues regarding the family are complex, as there are many emotions involved. Maintaining open communication with both the family and patient, and allowing grace and empathy for the individual, will allow them to live out the end of their life to the fullest.

Fibromyalgia is present in 2-8% of the population, so it is an important condition to be able to treat conservatively as future chiropractors. Fibromyalgia is classically accompanied by fatigue, memory problems, and sleep disturbances. Fibromyalgia causes chronic widespread pain and is an underdiagnosed condition. It is best managed as a chronic disease in which the primary care physician and other associated providers provide comprehensive care and continuous management of symptoms.

Risk factors associated with the development of fibromyalgia include genetic factors, female sex, and the presence of other painful conditions. Other painful conditions, such as rheumatoid arthritis and systemic lupus erythematosus are strongly associated with fibromyalgia, potentially due to the secondary central sensitization. Modifiable risk factors that may predispose individuals to the condition include sleep disturbances, physical inactivity, and obesity.

The clinical features of fibromyalgia are that the patient has chronic pain (>3 months) that is generalized and occurs in multiple sites, and that the pain is associated with fatigue, sleep problems, or cognitive/somatic symptoms. The individual may present with the chief complaint of 鈥淚 hurt all over,鈥 which is a classic statement from an individual with fibromyalgia. Additionally, some patients report cognitive problems affecting their memory, attention, and ability to focus or concentrate, commonly referred to as 鈥渇ibro fog.鈥

The overall approach to treating fibromyalgia should include maintaining function, improving quality of life, and the maintenance of symptoms. It is important to diagnose coexisting conditions, such as sleep disorders and major depressive disorder. Engagement in regular physical activity is crucial in the effective management of fibromyalgia. It is also crucial to educate the patient on the importance of sleep in moderating pain, fatigue, and cognitive symptoms. The utilization of acupuncture may provide benefits in terms of fatigue, pain, and well-being. Evidence is limited on the effectiveness of chiropractic manipulation, and more research is necessary. As fibromyalgia is incurable and chronic, conservative management is necessary for these patients to have the best quality of life.

Klippel-Feil Syndrome is a rare disorder that we learn about in many of our radiology classes. It is when two or more vertebrae in the neck are fused together from birth. People diagnosed with this syndrome may present with a short neck and limited movement, along with pain.

The most common symptoms of Klippel-Feil syndrome include a short neck and low hairline at the back of the head, limited movement affecting primarily the face, neck, upper body, and back, pain located in the head, neck or back, and hearing loss. Mutations in the GDF6 or GDF3 genes are associated with the development of this disorder. Tests can be performed at birth to determine if the disorder is mild or more severe, which will help determine the necessary treatment.

Diagnostic imaging examples of Klippel-Feil Syndrome and Sprengel Deformity, differing views.

Sprengel deformity is commonly associated with Klippel-Feil Syndrome, and is a rare congenital disorder in which the scapula is too high on one side of the body. The main signs and symptoms of Sprengel deformity are limited or restricted movement of the arm and shoulder blade, as well as the cervical spine. Some individuals may present with neck deformities as well, ranging from torticollis to severe spine deformity. CT scans and MRI are commonly used to confirm a diagnosis of Sprengel deformity.

The treatment for Sprengel deformity depends on the severity of the abnormality. For mild deformities, nonsurgical options such as chiropractic care and physical therapy are most beneficial. Moderate and severe deformities are candidates for surgical intervention. Although both Klippel-Feil syndrome and Sprengel deformity are rare disorders, it is important to have knowledge regarding the treatment of both, as more mild individuals may come into our office for conservative care.

The Salter-Harris classification system is utilized to grade fractures that occur in children and that involve the growth plate. The system grades features according to the involvement of the physis, metaphysis, and epiphysis. Most of the injuries that occur with the growth plate happen during growth spurts, as the physis is the weakest during this period of time. Active children are the most likely to encounter growth plate injuries, as ligaments and joint capsules surrounding the growth plate tend to be more strong and stable.

Once the physis fuses and the individual matures, ligamentous and tendon soft tissue injuries become more frequent.

A Salter Harris Type I fracture is when the fracture line extends through the physis or within the growth plate. An example of this type of fracture is Slipped Capital Femoral Epiphysis.

A Salter Harris Type II fracture is when the fracture extends through both the physis and metaphysis. These growth plate fractures are the most common and occur away from the joint space.

A Salter Harris Type III fracture is an intra-articular fracture that extends from the physis into the epiphysis.

A Salter Harris Type IV fracture is a fracture that passes through the epiphysis, physis, and metaphysis.

And last, a Salter Harris Type V fracture is due to a crush injury to the growth plate. This fracture pattern tends to result from severe injury, and leads to the poorest prognosis.

It is important to be aware of these fractures, as many complications can arise if they are not detected and treated quickly. Prolonged pain, mobility restrictions and growth restrictions are serious outcomes that can negatively influence an individual鈥檚 life, so it is important to be aware of the various growth plate fractures as future chiropractic physicians.
